A community health survey is being delivered to randomly selected Brown County residents via the US mail. The printed survey packet is a followup to the Community Health Survey postcard that was sent out in early August.

For those who received the postcard invitation to complete an online survey and have not had the opportunity to complete it, this is another opportunity to give Brown County Public Health your feedback. This survey gives community members an opportunity to tell us how conditions in Brown County affect your health and well-being.

The information gathered from this anonymous survey will help identify the challenges our community faces and will shape the work that Brown County Public Health and other community partners complete.

This survey will also help inform a Community Health Assessment, which assists Brown County Public Health and community partners in facilitating the creation of strategies that could improve and preserve the overall health and wellness of county residents.

The survey takes only a few minutes, and the information that is gathered will greatly benefit the community.
